EMR Systems

In many EMR systems, you will see additional chart notes, which include orientation and mood. To document, how does the patient present themselves in your office: normal or abnormal? And any related conditions: was today's visit related to an employment accident, auto accident, or another accident? Be sure to include the state where it occurred, as well as the date, and document that within the patient's chart.
